changes have been made to the courier address given
in the Electrical Inspection Order form (step 16 in the section entitled
“Instructions for Completing the Form”). As a consequence, the Electrical
Inspection Order form contained in Appendix B of the Office of the Fire Marshal
guideline TG 02-2003,
Orders, Orders to Close and Immediate Threat to Life,
has been revised. The revisions affect both the English and French versions of
the form.
The new courier address is:
400 Sheldon Dr., Unit 1, Cambridge, Ontario, N1T 2H9
the mailing and e-mail addresses remain the same.
Please replace all previous versions of Appendix B (English and French) and any
other existing forms with the old courier address with the revised form. Copies
of the English and French forms are attached for your convenience.
Persons using the OFM Microsoft Word form can change the address themselves or
obtain the file with the new courier address by sending a request by e-mail to
Elayne.Byng@jus.gov.on.ca.
