Municipal Fire Protection Information Guide and Survey
The survey form has been provided in two formats. The PDF format has been used to provide access to the document for those Fire Departments that do not have access to Microsoft Word. However, if the department wishes to electronically complete the survey, it can only be accomplished using the Microsoft Word version of the survey document.  The HTML version contains all the information of the PDF or Word version but no formatting.

Note: It is suggested that if you wish to complete the form electronically to download and save to your hard drive.
  • Tab to any field
  • Date format (yy-mm-dd)
    To ensure dates maintain the proper format, the "short date format" should be set to yyyy-mm-dd.
    In most Windows operating systems, "short date format" can be set by going to Start Menu > Settings > Control Panel > Regional Options > Date tab > short date format = "yy/mm/dd" > OK.
  • Drop down menu (yes/no)
  • Number format (use style 1,2,3 ....)
  • Currency - automatically formatted (ie; type 123 and will be formatted to read $123.00)
  • Help available (and visible) in task bar at bottom left of screen or by selecting F1 on your keyboard (similar message included at both locations).
