The Freedom of Information and Protection of Privacy Act has two purposes:

  • It gives people the right to see records and personal information that are under the custody or control of Ontario government ministries and agencies.
  • It helps protect every individual’s right to privacy. The Act tells government ministries and agencies what they can do and must do when they collect, use, disclose, retain and dispose of personal information.

What you need to know to make a request

  • If you are making a request for information under the Act, you’ll need to do it in writing. We do not accept requests through our website.
  • To file a request for information under the Act, please use the Access Request Application Form. There are two ways to get the form:
  • After you’ve filled out the form, you can send it in by mail or by fax. The mailing address and fax number are at the top of the form.
  • If you are asking for access to personal information on behalf of someone else, that person needs to complete the Authorization for Release of Personal Information Form - PDF, 13.7 kb. Please send the completed authorization form with your access request form.
  • When you file a request for information, please provide specific details about which records you want to see. If you need to know the types of records the ministry has, check the Directory of Records.
  • When you file a request for information there is a $5 application fee payable to the Minister of Finance. You will need to pay that fee when you send in your request. You can pay by cheque, money order or credit card (Visa or Master Card).
