This fire investigation report examines a fatal fire in a residential hotel called the Mizpah Hotel in Reno, Nevada. This four and five-storey building was a mixed use occupancy containing both hotel and residential suites as well as six commercial establishments on the ground floor. The severity of this arson fire was greatly exacerbated by a number of mattresses distributed along the corridor for replacement in various rooms. Fire spread was rapid and flashover quickly engulfed the second floor and ultimately caused the death of 12 victims and injuries to another 30 residents. The report focuses on issues of egress and escape since the mattresses impeded the movements of fleeing victims, and egress stairwells and fire escapes were not compliant with the NFPA Life Safety Code. The report also contains the contents of the investigation into the Paxton Hotel Fire in Chicago for comparisons on issues of egress design.

A series of explosions in a large grain storage facility in Haysville, Kansas, resulted in the deaths of seven people. The grain elevator was one of the largest in the world. The facility contained 246 concrete silos with each silo holding approximately 70,000 bushels of grain. The facility measured over 823 m or approximately on-half mile in length. Wheat was the main product being stored in this facility. An unknown ignition source ignited dust within the facility and resulted in a series of explosions killing seven workers and severely damaging the entire structure.
