Proceedings Commenced by Certificate of Offence: Revised Forms and OFM Technical Guideline

This is a reminder that effective July 1, 2009, fire services must implement
use of revised forms when initiating proceedings under Part I of the Provincial
Offences Act as the older “Certificate of Offence”, “Offence Notice” and
“Summons” forms become invalid on that date. These changes were adopted
on January 1, 2009, through amendments to Regulation 950 (R.R.O. 1990),
Proceedings Commenced by Certificate of Offence, made under the Provincial

Please note also that the Office of the Fire Marshal (OFM) technical guideline
OFM-TG-01-2006 has been withdrawn and replaced with a revised document OFM-TG-02-2009
"Commencing Proceedings Under Part I of the Provincial Offences Act" to
reflect the amendments to Regulation 950. The revised guideline can be accessed
